    This User Screening Form will help determine if you are a candidate for testing our innovative skincare formulation, which has been specifically developed to address multiple skin concerns on your hands and feet. Concerns may include dryness, cracking, burning sensations, redness, blistering, and more. It does this by:

    • Soothing, cooling and hydrating irritated skin on your hands and feet
    • Moisturizing and softening the skin

    In addition, our formulation is gentle on the skin, has been clinically tested and passed rigorous safety standards.

    The purpose of this product survey is to measure the improvement of symptoms over a 4-week period and its impact on the overall quality of life.

    The information you provide will be kept strictly confidential and will only be used to assess your eligibility for participation in this product survey.
